The Health Insurance Portability and Accountability Act (HIPAA) provides you with important rights regarding your protected health information (PHI). As our patient, you have the right to: access and obtain a copy of your health records; request corrections to your records; receive an accounting of disclosures of your PHI; request restrictions on certain uses and disclosures; and receive this Notice of Privacy Practices.
We use and disclose your protected health information for treatment purposes (coordinating your care), payment purposes (billing your insurance), and healthcare operations (quality improvement, administrative functions). These uses are permitted under HIPAA without requiring your specific authorization.
We will obtain your written authorization before using or disclosing your PHI for purposes other than treatment, payment, or operations — such as marketing, sale of PHI, or psychotherapy notes. You may revoke any authorization in writing at any time.
We maintain administrative, physical, and technical safeguards to protect the privacy and security of your health information. All staff members are trained on HIPAA requirements and our internal privacy policies.
When using or disclosing PHI, we make reasonable efforts to limit access to only the minimum amount of information necessary to accomplish the intended purpose.
To access your records, request amendments, or exercise any other HIPAA rights, please submit a written request to our office. We will respond within 30 days as required by law. We may charge a reasonable fee for copies of records.
If you believe your privacy rights have been violated, you may file a complaint with us directly at imcarednp@gmail.com or with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr. You will not be penalized for filing a complaint.
We reserve the right to change this Notice of Privacy Practices. Any revised notice will apply to all PHI we maintain. The current version will always be available at this office and on our website.
